□ Certificates from certifying agencies or groups/organizations

Service Providers:
Standard Service Provider Questionnaire
□ Company health, safety, security, environmental and insurance information
Insurance
□ General Liability - minimum $1,000,000 per occurance
□ Employee Liability - minimum $1,000,000 per occurance
□ Automobile Liability - minimum $1,000,000 per occurance
□ Workers Compensation
Health and Safety
□ Health and Safety Programs
□ Occupational Safety and Health Administration (OSHA) logs for the last 3 years
□ Employee Modifier Rate (EMR) letters for the last 3 years

Security
□ Background Check Information
□ In late 2008 or early 2009, contractors doing work in refinery and terminal locations will be required to have a Transportation Worker Identification Card (TWIC) issued by the federal government for entry.
